Information collected

The support enquiry form asks for contact details, your Greater Sydney area, the services you are interested in and a short summary. The feedback form lets you remain anonymous or provide contact details if you would like a response.

Why ACE collects it

Information submitted through the website is used to respond to enquiries, referrals, callback requests, feedback and complaints. The forms are intentionally designed not to request detailed medical records.

How form information is delivered

The intended launch setup sends submissions to ACE’s nominated business email inbox and sends a confirmation to the person when they provide an email address. The website is not intended to keep a separate long-term database of form submissions.

Your choices

You may contact ACE by phone instead of using a form. You may also submit feedback or a complaint anonymously. Anonymous submissions cannot receive a direct response.
